Death Grant

On the death of a secured person receiving or entitled to receive injury benefit, sickness benefit or medical care at the time of his death or while he is in receipt of a total disablement pension, the surviving widow/widows, or needy widower, or if there be no surviving widow/widows or needy widower, the person who provided for the funeral, shall be entitled to a death grant equal to the daily rate of sickness benefit multiplied by thirty, but in no case less than Rs.15,000/-.

 

DOCUMENTS REQUIRED:

  1. Form B-7 for claiming the death grant duly completed in all respects and attested by the employer concerned, from the widow(s)/needy widower/legal heirs.
  2. Computerized death certificate of the deceased secured worker.
  3. Original Social Security Registration Card (R-5) / Benazir Mazdoor Card.
  4. Certificate of employment by the employer on form B-2.
  5. Marriage certificate or other acceptable evidence of marriage (in case the claimant is widow).
  6. Evidence of cost of funeral expenses incurred by the claimant.
  7. Affidavit taking oath for deserving the death grant.
  8. Any other evidence and identity of the claimant required by the Institution.

 

TIME LIMIT:

The claim should be filed within 15 days of the date of death of the secured person.

 

ENTITLEMENT:

The surviving widow/widows, or needy widower, or if there be no surviving widow/widows or needy widower, the person who provided for the funeral, shall be entitled to receive death grant.

 

RATE:

Rs.15,000/- or equal to the daily rate of sickness benefit multiplied by thirty, whichever is more.
